The initiative Coordinate My Care, through which healthcare professionals, once granted permission, could share a patient’s urgent care plan with other professionals, has been embraced by a number of London hospitals, including Kingston and Richmond, and will be promoted as Coordinate Your Care. In the future, these hospitals will also work with other health and social care providers to share a more detailed care record of patients, which will include treatment received from community NHS services, mental health services and social care services.
If you are happy for your information to be shared in this way, you don’t need to do anything. If you do not want your information to be shared, you can opt-out or pick up a form at your GP’s surgery.
